Ultrasound report at 8, 9 and 10 weeks: ready templates
Between 8 and 10 weeks the embryo already has a heartbeat and movements, and the CRL gives the most accurate dating of the whole pregnancy. Below are the singleton template, the dichorionic twin template, since this is the best stage to determine chorionicity, and the example of an embryo without a heartbeat, with the pregnancy failure criterion written in the report.
TRANSVAGINAL OBSTETRIC ULTRASOUND
TECHNIQUE:
Transvaginal examination.
FINDINGS:
Anteverted, anteflexed uterus with regular contours, containing an intrauterine gestational sac with corporal implantation.
Yolk sac present, with normal appearance.
Single embryo, with a crown-rump length (CRL) of [x] mm.
Embryonic cardiac activity present, with a rate of [x] bpm.
Embryonic movements present.
No signs of subchorionic hematoma.
Ovaries of normal size and appearance. Corpus luteum in the [right/left] ovary.
No free fluid in the pelvis.
IMPRESSION:
Single intrauterine pregnancy with a live embryo.
Gestational age by CRL: [x] weeks and [x] days.
Estimated due date: [dd/mm/yyyy].
FINDINGS:
Uterus containing two intrauterine gestational sacs, separated by a thick membrane with a lambda sign.
Gestational sac [1]: embryo with a CRL of [x] mm and cardiac activity present, heart rate of [x] bpm.
Gestational sac [2]: embryo with a CRL of [x] mm and cardiac activity present, heart rate of [x] bpm.
Yolk sac present in each gestational sac.
Ovaries of normal size and appearance.
No free fluid in the pelvis.
IMPRESSION:
Intrauterine dichorionic diamniotic twin pregnancy with two live embryos.
Gestational age by the CRL of the larger embryo: [x] weeks and [x] days.
Estimated due date: [dd/mm/yyyy].
FINDINGS:
Intrauterine gestational sac, mean sac diameter of 28 mm, containing a yolk sac.
Single embryo, CRL of 12 mm, corresponding to 7 weeks and 3 days.
No embryonic cardiac activity is identified.
Embryonic movements absent.
IMPRESSION:
Intrauterine pregnancy with a 12 mm embryo without cardiac activity.
Finding consistent with pregnancy failure, by the criterion of a CRL of 7 mm or more without cardiac activity.
Fields in brackets are filled in from the exam. The template is a starting point: the report must reflect what was seen and your practice's guidelines.
