Ultrasound report at 5, 6 and 7 weeks: ready templates

Between 5 and 7 weeks the scan changes from one week to the next: at 5 weeks you expect the gestational sac and the yolk sac, still without an embryo; at 6 to 7 weeks, the embryo with cardiac activity. Below are the template for each stage and the example of an embryo still without a heartbeat, which is the hardest report to write.

6 to 7 weeks template: embryo with heartbeat

TRANSVAGINAL OBSTETRIC ULTRASOUND

TECHNIQUE:

Transvaginal examination.

FINDINGS:

Anteverted, anteflexed uterus with regular contours, containing an intrauterine gestational sac implanted in the uterine body, with a regular decidual reaction.

Mean gestational sac diameter: [x] mm.

Yolk sac present, normal in appearance, measuring [x] mm.

Single embryo, with a crown-rump length (CRL) of [x] mm.

Embryonic cardiac activity present, with a rate of [x] bpm.

No signs of subchorionic hematoma.

Ovaries of normal size and appearance. Corpus luteum in the [right/left] ovary.

No free fluid in the pelvis.

IMPRESSION:

Single intrauterine pregnancy with a live embryo.

Gestational age by CRL: [x] weeks and [x] days.

Estimated due date: [dd/mm/yyyy].

5 weeks template: gestational sac and yolk sac

FINDINGS:

Anteverted, anteflexed uterus with regular contours, containing an intrauterine gestational sac implanted in the uterine body, with a regular decidual reaction.

Mean gestational sac diameter: [x] mm.

Yolk sac present, normal in appearance, measuring [x] mm.

Embryo not yet identifiable, an expected finding for the gestational age.

Ovaries of normal size and appearance. Corpus luteum in the [right/left] ovary.

No free fluid in the pelvis.

IMPRESSION:

Early intrauterine pregnancy, consistent with [x] weeks by last menstrual period.

No embryo identifiable yet, so viability cannot be determined on this scan.

RECOMMENDATIONS:

Follow-up ultrasound in 7 to 14 days to confirm the embryo and cardiac activity.

Example: embryo still without a heartbeat

FINDINGS:

Intrauterine gestational sac, mean sac diameter of 15 mm, containing a 4 mm yolk sac.

Single embryo, CRL of 4 mm.

No embryonic cardiac activity is identified at the time of the scan.

IMPRESSION:

Intrauterine pregnancy with a 4 mm embryo without identifiable cardiac activity.

This finding does not meet the criteria for pregnancy failure (CRL less than 7 mm).

RECOMMENDATIONS:

Follow-up ultrasound in 7 to 10 days.

Fields in brackets are filled in from the exam. The template is a starting point: the report must reflect what was seen and your practice's guidelines.

Frequently asked questions

Is the embryo visible on ultrasound at 5 weeks?
Usually not. At 5 weeks the gestational sac is seen and, from about 5.5 weeks, the yolk sac. The embryo with a heartbeat usually appears around 6 weeks on transvaginal scanning.
How do you estimate gestational age before the embryo appears?
A practical estimate is gestational age in days equal to the mean sac diameter in millimeters plus 30. Once the embryo appears, dating is by CRL.
What heart rate is expected at 6 and 7 weeks?
Around 6 weeks the heart rate is usually between 100 and 120 bpm and rises over the following weeks. A rate below 100 bpm at this stage warrants follow-up.
Is an embryo without a heartbeat at 6 weeks a miscarriage?
Not necessarily. Below a CRL of 7 mm, absent cardiac activity is suspicious but does not define pregnancy failure: the report describes the finding and recommends follow-up in 7 to 10 days.
