Ultrasound report at 11, 12, 13 and 14 weeks: ready templates

Between 11 and 14 weeks the obstetric scan confirms viability, the number of fetuses and gestational age, and it is the last stage in which the CRL dates the pregnancy accurately. Below are the singleton template, the monochorionic twin template and the example of a gestational age that differs from the LMP. Nuchal translucency and aneuploidy screening are in the first-trimester anomaly scan template.

11 to 14 weeks template: singleton

OBSTETRIC ULTRASOUND

TECHNIQUE:

Transabdominal scan.

FINDINGS:

Gravid uterus containing a single fetus.

Fetal cardiac activity present, heart rate of [x] bpm. Fetal movements present.

Crown-rump length (CRL): [x] mm.

Cephalic pole with an ossified skull vault. Stomach and bladder visible. Four limbs identified.

[Anterior/posterior] placenta, with normal appearance.

Normal amniotic fluid volume.

IMPRESSION:

Single pregnancy with a live fetus, [x] weeks and [x] days by CRL.

Estimated due date: [dd/mm/yyyy].

NOTE:

This scan does not include aneuploidy screening (nuchal translucency and other markers), which is done in the first-trimester anomaly scan.

11 to 14 weeks template: monochorionic twins

FINDINGS:

Gravid uterus containing two fetuses, separated by a thin membrane with a T sign.

Fetus [1]: CRL of [x] mm and cardiac activity present, heart rate of [x] bpm.

Fetus [2]: CRL of [x] mm and cardiac activity present, heart rate of [x] bpm.

Single [anterior/posterior] placenta.

Normal amniotic fluid volume in both sacs.

IMPRESSION:

Monochorionic diamniotic twin pregnancy with two live fetuses.

Gestational age by the CRL of the larger fetus: [x] weeks and [x] days.

CRL discordance: [x]%.

RECOMMENDATIONS:

Ultrasound follow-up every 2 weeks from 16 weeks, due to the risk of twin-to-twin transfusion syndrome.

Example: gestational age different from the LMP

FINDINGS:

Single fetus, cardiac activity present.

CRL of 52 mm, corresponding to 11 weeks and 5 days.

Gestational age by LMP: 13 weeks.

IMPRESSION:

Singleton pregnancy with a live fetus.

Gestational age by CRL of 11 weeks and 5 days, 9 days less than calculated by LMP.

Difference greater than 7 days: dating is now by CRL, with an estimated due date of [dd/mm/yyyy].

Fields in brackets are filled in from the exam. The template is a starting point: the report must reflect what was seen and your practice's guidelines.

Frequently asked questions

What is the difference between this scan and the first-trimester anomaly scan?
The 11 to 14 weeks obstetric scan confirms viability, number of fetuses and gestational age. The anomaly scan in the same window adds nuchal translucency, the other aneuploidy markers, early anatomy and the risk calculation.
When should the pregnancy be re-dated by CRL?
Up to 13 weeks and 6 days, when the difference between gestational age by CRL and by LMP is more than 7 days. From then on the due date follows the CRL and does not change on later scans.
What is the normal fetal heart rate at 12 weeks?
After peaking at about 170 bpm around 9 to 10 weeks, the rate gradually falls. Between 11 and 14 weeks it is usually between 140 and 170 bpm.
How do you tell dichorionic from monochorionic twins?
By where the membrane meets the placenta: in the lambda sign placental tissue extends between the membranes and the pregnancy is dichorionic; in the T sign the thin membrane inserts at a right angle and the pregnancy is monochorionic.
Can the baby's sex be seen at 12 weeks?
Sometimes, by the angle of the genital tubercle, with accuracy improving from 13 weeks. The most reliable confirmation is on the second-trimester scan.
